In Feb 2007, the keel of a 35ft yacht became detached, causing the boat to capsize, resulting in loss of life. The subsequent UK Marine Accident Investigation Board (MAIB) found that the fabricated steel keel had failed just below the fillet weld connecting the fin to the taper box. The MAIB Report identifies a number of design, inspection and repair issues which contributed to the failure. The full report is available on the MAIB website at www.maib.gov.uk
